Social Security Information

A social security number is an important document that is used for identification, tax, and employment purposes. In order to apply for a Social Security number, all applicants need at least two documents as proof of your age, identity, and U.S. citizenship or lawful alien status.

For a F-1, M-1 and J-1 student, you have to submit all four of these documents:

1. A letter from the international student advisor stating you are authorized to work on campus.
2. Passport
3. INS Form I-20 (or DS-2019)
4. Form I-94 (which is a departure record, needed to enter United States and usually is stapled into your passport)

*All persons applying for a Social Security number require original or certified copies of all documents requested above. Photocopies are not acceptable.

Also, you need to complete Form SS-5 to apply for the number. Form SS-5 is available for download at www.ssa.gov/online/ss-5.html. Or you can obtain the form by visiting International Student Services in 7 Havican Hall, or your local Social Security office.
