Steven L. Posar, M.D., Founder and CEO of GuideStar Eldercare, is a nationally recognized leader in geriatric neuropsychiatry and innovative dementia care. For more than 15 years, Dr. Posar and GuideStar Eldercare have delivered an unmatched level of medical care and clinical innovation for adults and older adults with complex neurological and behavioral health needs.

Dr. Posar recognized early the growing demand for integrated psychiatric, neurological, medical, and psychological care for adult and elderly patients with neurological and behavioral illness. He pioneered a neurology‑forward model that unites neurology, psychiatry, psychology, internal medicine, and geriatrics, transforming how neurocognitive disorders are evaluated and treated in long‑term care. Under his leadership, GuideStar Eldercare launched one of the nation’s first onsite consultative neurology services in long‑term care settings. His clinical innovations have produced measurable system‑wide impact, including a 68 percent reduction in antipsychotic use and a 75 percent decrease in psychiatric hospital admissions. A trained Gerontologist and experienced entrepreneur, Dr. Posar has elevated the standard of care for a population too often overlooked in traditional healthcare models.

Dr. Posar is President and Founder of the Steven L. Posar MD Eldercare Foundation, which advances research, education, and compassionate care for aging populations. He is also a Clinical Professor of Geriatric Neuropsychiatry at Saint Mary’s College, where he established the college’s certificate program in geriatric neuropsychiatry, expanding professional training pathways for clinicians dedicated to the care of older adults.

A respected national speaker, Dr. Posar presents regularly at PALTC, OHCA, McKnight’s, and other leading forums. His academic work includes published research on dementia care, medication optimization, and interdisciplinary treatment models.
