2019–20 Financial Aid Forms & Publications

Below you will find links to online financial aid forms you may need to complete for the 2019–20 academic year. Please complete ONLY the forms that have been requested by the Office of Financial Aid. Need help understanding the details of the financial aid award notices and awards? See these helpful guides.

While the verification forms can be completed with mobile devices, we recommend you complete the Verification Statements on your personal computer.


Prospective students and a parent will need to create their own separate Dynamic Forms accounts in order to complete the online forms. The Parent Verification Statement must be completed and signed by a parent with their own Dynamic Forms account information. The Student Verification Statement must be completed and signed by the student with her own Dynamic Forms account information. Incorrect forms will be rejected. 

Current students and parents who previously created a Dynamic Forms account will not need to create a new account. Current students will log into the Saint Mary's portal to complete the required student forms. Parents of current students will use their previously created account information to log in and complete the required parent forms. 

Instructions for Creating an Account for Online Forms

The student ID begins with 980 (for example, 980123456) and is listed on most communications from Financial Aid. 

If you did not complete the IRS data retrieval through the FAFSA and are attaching tax information to the verification statement, please note that we must have a tax RETURN transcript which you may print or order from www.irs.gov.
